Choose the form that you want to edit or delete.

You can now change or remove the consent form based on your preference.

1. Go to the "Setup".
2. Click on the "Consent Forms".
3. Choose the form that you want to edit or delete.
4. Click on the (...) dots.
5. You can now change or remove the consent form based on your preference.
