To Create the Consent form Follow the steps below :
Go to the "Setup".
Click on the "Create Form".
3. Enter the "Form Name".
4. Drag & Drop the required fields, Text Field, Email, Button etc.
5. Click on SAVE, Once its done.
After Saving the consent form, Click on the three dot (...) under "Action" button to Edit, preview or delete the form.
